a Process Safety Specialist within the
Health and Safety Executive
(HSE), the regulatory body for Occupational Safety and Health in
the UK. I took early retirement from HSE in 2007 but continued to
be active in consultancy and on behalf of the
Institution of Chemical Engineers
(IChemE),
Safety and Loss Prevention Special Interest Group and as an IChemE
'ChemEnvoy' available to speak to the public, media etc. on
safety in the Process Industries for the following decade.
I am now fully retired.
The Safety and Loss Prevention Special Interest Group web site includes a wide variety of technical papers
available for free download. These include papers from
28 conferences in the 'Hazards' series,
171 issues of Safety Newsletters produced by the former ICI,
Safety and Loss Prevention Teaching Notes, and
a selection of old or out-of-print accident reports produced by the Health and Safety Executive.

'Process Safety - Responsibility and Accountability'

I have given some of the presentations at the
Fundamentals of Process Safety course, organised by IChemE.
My focus is on the numerical aspects of quantitative risk assessment.
Course delegates often often struggle with the concepts of individual and
societal risk and the relationship between them. In order to clarify this
topic area, I have produced two short notes giving further explanation
by way of examples. These notes are:
I have also produced an FN plot showing relevant criteria and several important data points.

An additional aspect of societal risk that also causes confusion is the use of
Risk Integrals by the UK HSE . I have produced a short explanatory note on this topic:

site badged by Lloyd’s Register and operated by Paul Davies) to provoke
debate about safety and loss prevention issues.
